If you or the subject has moved then we would say “blurred”. (as others have said)
If the focus ring on the lens has not been set correctly for “sharp” edges then we would say “out of focus”. (As you have said)
If there are other problems then there are other descriptions.
Overexposed, washed out — stopped too low.
Underexposed — stopped too high.
Blurred foreground and background with sharp subject – stopped too low.

There can also be problems with development – of course, not if you’re shooting digital, then you just fix it with Photoshop!
